The Department should follow the established procedure at the University level for reporting and investigating accidents/incidents. The "HKUST Incident/Accident Report Form" should be used for the reporting. For accident/incident that warrants more thorough investigation, a separate investigation report may be necessary as an attachment to the Form. 

Apart from reporting the accidents/incidents according to the University-wide procedure. The Department may also need to establish its own internal procedure for the same to suit the need of the Department in maintaining a better control of the appropriate follow-up efforts.
