This Personal Information Collection Statement (“PICS”) is provided by the Health, Safety and Environment Office (“HSEO”) of The Hong Kong University of Science and Technology (“HKUST”) for the purposes of complying with the notification requirements under the Personal Data (Privacy) Ordinance of Hong Kong (“PDPO”) when collecting personal data. It should be read in conjunction with HKUST’s Data Privacy Policy Statement (“PPS”), a copy of which is available at this link: https://www.ust.hk/privacy-policy/.

The personal data collected from you, including your Staff/Student ID, your enrollment data, medical exam records and test, monitoring or assessment results of the hazardous conditions or agents to which you have been exposed or may be exposed, will be provided to and used by relevant Departments/Organizations within or outside HKUST for the occupational health assessment and medical surveillance requirements outlined in the HKUST Safety and Environmental Protection Manual - Chapter 12 - Section M, and related purposes. The HSEO will handle all personal data with strict security and confidentiality, and in compliance with the PDPO and PPS. Personal data will be kept only for as long as necessary to fulfil the aforementioned purposes, and will be destroyed once it is no longer required.

You may request access to and correction of your personal data in the records of the HSEO. Such requests may be sent to safety@ust.hk.

The HSEO will handle all personal data with strict security and confidentiality, and in compliance with the PDPO and PPS. Personal data will be kept only for as long as necessary to fulfil the aforementioned purposes, and will be destroyed once it is no longer required.
