HSEO investigates all reported accidents/incidents. However, this should not be a substitute for the investigation efforts made by the responsible staff (normally the Supervisor and/or the DSO) in the Department. Except for more complicated cases, the findings and recommendations can be written in the appropriate fields in the "HKUST Incident/Accident Report Form"
